The Importance of Community-Based Case Management

“The healthcare delivery system, while technologically advanced and with more of an eye today toward improving outcomes and patient safety, I think it’s still much in need of the kind of personal and professional connection of a caring case manager. I think we have the ability to meet that need, and out commitment to patients and passion for the process of case management will enable us to serve these patients one patient at a time.”
